Add a link to our GitHub issue tracker to collect feedback

Add a button for submitting feedback via our GitHub repository on the webpage footer. When creating a ticket, users should be presented with one or more GitHub templates to ensure that we are sufficiently collecting their feedback. Possible templates may include "feature enhancement", "bug", ...
