Random unexpected nil disconnect error on iOS

After upgrading to 1.16.1 on iOS, my clients and I frequently face this error type.

Previously, it only occurred when the software was upgraded with an active VPN connection; now it happens very randomly and also frequently.

ProxyConnectionFailure: Failed to connect to server Server-VPN-B#1.
Cause:   ERR_INTERNAL_ERROR: unexpected nil disconnect error

Sometimes it can only be fixed by removing the VPN profile through iOS Settings > General > VPN & Device Management > VPN. Then, rebooting the device.

Sometimes, it can be fixed by killing the app from the recent menu but this method does not always work.

Steps to reproduce

I am not sure how to reproduce.

One way to reproduce is to update the Outline with an active VPN connection.

Yes, this is a known issue. "Removing the VPN profile. Then, rebooting the device." is the recommended solution.

We're looking into why this happens. It appears to be an issue where iOS/macOS occasionally fails to load the correct VPN extension from our app, causing the connection to fail.
