Config file for surveys to appear

Instead of hard coding a link to the survey, need to create a config file to pull the survey link from to put into the iframe. This way people can create their own surveys if they want.

Need to update the frontend to not only look at the config file for the survey link and if there is no survey link, we should remove the survey button from the frontend and update all protocol pages to not go to the survey page if no survey is configured.
