HTML Table ext for DMZ

Extension inspired by DMZ HTML Form. Generates table in a similar way.

Features are:

• label from $validation / custom label
• mapping by $validation['field']['values'] / custom array mapping
• custom function-generated fields
• autopagination with native CI paginator when _paged Docs are inside of distrib.
