Memory-bank needs a clear path to store files

Sometimes i see it being created in .github and other times it created in the middle of source code so my sugestion is that it needs a reference.

I added The Memory Bank is located in the `.github/memory-bank/` folder at the project root. This is not optional you MUST always look here first. to the file memory-bank instructions and it seems to do the trick but after playing with it i dont believe this memory feature is really working as intended or i am not doing it correctly.

I used it on a ongoing project and it hallucinated at the begining with task TASK001, TASK002 and TASK003 since i dont have any of those features in this project. After giving it a few tasks to work and at the end tell it to update memory banks it created TASK009 that is correct. During this time i never saw it load as context any of the memory bank files, only the instructions files. This was during 3 days using Github Copilot and alternating between GPT 5 mini and Claude 4 models.
