Update Github templates

Let's add issue templates for better ticket details. Not sure if the standard format (bug report, feature request) works here since we're using issues to track tasks. Will look into it.

We should also update our pull request template to reduce developer friction in filling it out, and switching ticket tracking from Jira to Github.
