Use the relative_url filter to properly format URLs instead of manually doing it.

The previous method didn't work well with Github Pages in custom domains, since posts in a subdirectory (ie,
https://example.com/dir/post.html) linked their assets without the leading slash, trying to load
https://example.com/dir/assets/css/screen.css instead.
