Update Claude Code integration guide to reflect new hook architecture and link code/commits to Langfuse traces.

The previous guide only covered a single Stop hook. This update documents the full hook stack (session init, stop, post-Bash commit, prepare-commit-msg) and explains how LANGFUSE_PROJECT_ID and commit trailers are used to establish a deterministic link between Claude-generated code, git commits, and Langfuse traces/sessions. This linkage provides insights into agent performance, cost, and development process.
