- Let's add an assert to test_codex that checks every request we get for dangling items.
- I'd prefer operations on history to be individually correct and not rely on huge fallback to clean up after them.
- Let's keep the fallback for now but make it panic in debug.

We are doing some ad-hoc logic while dealing with conversation history. Ideally, we shouldn't mutate
vec[responseitem]manually at all and should depend onConversationHistoryfor those changes.Those changes are:
I am also adding some
errorlogs for cases we shouldn't ideally face. For example, we shouldn't be missingtoolcallsoroutputs. We shouldn't hitContextWindowExceededwhile performingcompactThis refactor will give us granular control over our context management.
