Adds benign_application_error sample (worker, starter, activity, workflow, README).
Demonstrates ApplicationError(category=ApplicationErrorCategory.BENIGN):
BENIGN → activity failure logged at DEBUG, no activity-failure metrics
Non-BENIGN → WARN log, metrics emitted
Includes JSON logging + SDK log forwarding via LogForwardingConfig.
