feat: multiple updates

Description

• Doc cleanup
• Removed duplicate inputs from ibm_catalog.json (this is now caught in updated pre-commit hook)
• updated the standard variations to use Elasticsearch platinum plan, b3c.4x16.encrypted host flavor, 3 dedicated CPUs, and deployment of the elsr model. These all used to be in the the standard varitation but seems were incorrect removed in v4.
• Bumped to latest RAG sample app DA which has several fixes in it
